FALLBROOK – The Fallbrook Healthcare District has pledged and delivered a portion of its revenues back into the community through the funding and sponsorship of programs and services that benefit the health and well-being of the residents of the district. Two hundred and five grants have been extended to 38 non-profit agencies who serve children, adults and senior citizens in Fallbrook, Bonsall, Rainbow and DeLuz.

Budgetary considerations of the district remain the determining factor in funds available for grants. The Board of Directors has defined a limit of $555,000 for grant year 2013-2014. It remains committed to identifying, promoting and supporting a broad range of healthcare related needs within the district.

There are seven phases in the grant program process, beginning with the Request for Proposal – the RFP.

The RFP should be a brief, but concise, description of the proposed program, its target population, its benefit and its cost. It should also indicate support of their own organization’s mission as well as that of FHD. The program proposed must be focused in health related in areas of prevention/education, treatment, and/or ancillary services and must be delivered by non-profit agencies benefitting residents of the Fallbrook, Bonsall, Rainbow and DeLuz areas.

FHD will not fund the following: activities in direct competition with those provided by Fallbrook Hospital; endowments; expenses related to fundraising or lobbying of public officials or other political purposes; organizations intending pass through or regranting of district funds to other organizations; basic research defined as the pursuit of knowledge without immediate practical program or human applications; sectarian purposes; individuals; replacement funds so that a current funding can be shifted to other programs of the applicant; programs related to provision of housing, employment opportunity and/or educational pursuits for the purpose of employment.

All qualified agencies that secure a packet and submit an RFP will be considered. The packet includes a listing of the grant phases and guidelines inclusive of eligibility and criteria. Deadline for submissions is Wednesday March 20.

RFP’s will be reviewed by the Directors of the Board with acceptance or decline determined in the Board meeting of April 10, 2013.

If the RFP is accepted, an invitation to present a full grant application will be extended by the board. If the RFP is declined, the agency will be notified and no invitation to apply will be extended.

The full grant application will require a more detailed presentation of the proposed program inclusive of project goals, project budget, project narrative and financial statements of the organization with two year comparative balance sheet.

Completed grant applications will be due in the district administrative office on Friday, May 3. Only invited grant applicants are accepted and considered. The board’s final review and selection of grant recipients will be at the board meeting of Wednesday, June 12. The grant year runs July 1, 2013 through June 30, 2014. Grant awards will be presented in the board meeting of July 11, 2013.

All recipients of grant funding are required to present periodic reports to the board inclusive of information specific to their funded program and the utilization of funds awarded for that program. Additionally, they are obligated to support and participate in other undertakings of the district’s healthcare programs.
